· He/she must receive letter of eligibility for DIPS retraining course by Traffic Police
· He/she must register and book a course date within 14 days from the date of this letter
· Course fee paid is non-refundable
· No cancellation is allowed upon course confirmation
· He/she must attend 2 training days, and must be completed and passed to receive a remission in the suspension period
· The course date can only start at least 3 days after the enrolment date

I agree and give consent to:

1.Allow CDC to collect,use and retain my personal data(including Digital Photograph) for the purpose of training and testing services by Traffic Police, training and payment matters. For safety and training purposes, CDC training cars may be equipped with In-Vehicle Camera (outward-facing) with both video and audio recording capabilities.

2.Allow CDC to disclose my personal data to third party users – Insurance Company, Government Authorities mainly Traffic Police for purpose of training and testing services.

3.Send or contact me via telephone, electronic mail, short message service (SMS) and mobile messaging app and postal mail regarding any changes to the training or testing services.
